Initial visits last 1 hour and are completed in-person. A comprehensive medical history will be completed along with pertinent physical exams. At our clinical discretion, we may order lab tests and/or imaging. You may receive a few initial recommendations to get your healing journey started.
How can I prepare for my initial visit?
An invitation to your Patient Portal and New Patient Questionnaire will be made available 24 hours before your scheduled appointment. In line with our efforts towards sustainability, the New Patient Questionnaire is designed to be completed electronically. If you prefer to complete this by hand, simply email us and we will send a PDF for printing.
Please allow 20 minutes to complete the Questionnaire. A complete New Patient Questionnaire at the beginning of the scheduled appointment allows us to maximize our time together.

How often are my follow-up visits?
Every 4-6 weeks. Frequency may vary based upon health condition, severity, and stage of treatment.
What can I expect at my follow-up visits?
Established patients can expect a comprehensive evaluation of treatment progress including review of labs tests, adjustments to medications/supplements, and on-going conversation of short- and long-term health & wellness goals.
How does telemedicine work?
When scheduling appointments through your Patient Portal, Established Patients may choose to complete follow-up consults via phone or video conferencing Established Patients are given access to test results via the Patient Portal so you can follow along.
Is telemedicine safe?
Yes – and convenient. Phone calls and video conferencing via your Patient Portal are HIPAA-compliant. Established patients may also send messages to Dr. Nagallo through the Patient Portal.

Do you bill insurance?
Unfortunately, health insurance plans do not cover preventative health services so we currently do not bill insurance. At the Established Patient’s request, we can provide a service summary (“Superbill”) for services rendered and products purchased that you may submit to your health insurance provider for reimbursement. Medicare, Tricare, and AHCCCS (Arizona Health Care Cost Containment System) currently do not cover services by naturopathic doctors. While many of our patients are successful in receiving reimbursement, we do not provide any sort of guarantee.
How do I pay?
We accept all major credit/debit cards (Visa, Master Card, American Express, Discover), cash, and cheques (written to Sustained Integrative Health). We also accept Health Savings Account (HSA) and Flex Spending Accounts (FSA) if your plan covers naturopathic services and vitamins/supplements. It is your responsibility to ensure your HSA or FSA plan covers our services and products.
